Your First Appointment
During the first visit at our clinic you will need to fill out our intake forms, which may take a few minutes. We also ask that you bring the following:
- Health history from childhood onward including sicknesses, hospitalizations, accidents, etc.
- A complete list of all physical symptoms like headaches, digestive problems, aches and pains, etc.
- If you are taking any medicines or supplements please bring the bottles with you.
Costs
- Our initial check up takes one to two hours and the cost of the checkup is $132.00. This does not include the cost of natural remedies.
- Follow-up visits are approximately 30 minutes and costs $47.00
